Insurance Company Regulations

An understanding of your insurance policy is considered your responsibility. If your insurance requires an authorization for specialists, please contact us at least 48 hours in advance. Managed care plans require that any visit to another facility must be authorized by your pediatrician. If you have any questions about your coverage, please contact your insurance company. If there is a need to refer you to a specialist, we will provide approved referrals. Our doctor may recommend procedures or tests that your insurance company may not pay for. You will be responsible for any fees that have not been approved by your insurance company. 

Fees and Insurances 

We kindly request payment in full for services rendered at the time of each visit. Payment may be made with cash, check, Visa, Master Card, or American Express. Co-payments or deductibles are collected at check-in. If your insurance plan is one that we accept, and the physician services are to be covered expenses, we will gladly file your claim. Please bring your current insurance card and picture ID with you. You will be billed for any amount that your insurance company leaves to your responsibility. Fees are due when services are rendered.
